Rheumatic heart disease

How would you code rheumatic heart disease with mitral stenosis, tricuspid regurgitation, aortic and pulmonic insufficiency status post commissurotomy? Thanks

It's an interesting situation. Please note that the different diagnostic conditions are not directly linked with rheumatic condition of heart. Therefore as per ICD coding rule, you cannot combine those Dx and cannot arrive at combo-codes. You need to code them separately, like this -- rheumatic heart disease (398.90), mitral stenosis (394.0), tricuspid regurgitation (397.0), aortic insufficiency (424.1), pulmonic insufficiency (424.3).

If all the Dx are linked directly with Rheumatic heart condition, then only you could use codes like 394.1, 397.1, 395.1 / 396.1, otherwise not.
