My doctor did an endoscopic carpal tunnel release and then took the patient back to the OR several days later to perform an open carpal tunnel release with a nerve conduit protector. I'm thinking 64721,58. The conduit (64910) bundles. Would this be correct?

Surgical treatment of carpal tunnel syndrome is reported using 64721 (neuroplasty and/or transposition; median nerve at carpal tunnel). For conduit code is 64910 but both these codes have CCI conflict. 64721,58 is correct.

Looking into this surgery further I noticed my doctor did not do a conduit nerve repair 64910 but just wrapped a contused nerve in a protector during the carpal tunnel release. Is there a code I can add to 64721 that will cover the application of the protector?

CPT guidelines for code 64721 Includes Neuroplasty with nerve wrapping and Includes Surgical decompression/freeing of nerve from scar tissue. So, nerve wrapping is included in 64721.
