revision of ptosis repair

Deaconess Posted Thu 03rd of November, 2016 14:23:22 PM
is there a code used for repair of a previously repaired blepharoptosis? I know 67909 is for revision of over-correction, but what about under-correction? the patient had surgery 3 weeks ago, and is now back in having the blepharoptosis with advancement of both levator and Muller's muscle.

Since, a major surgery was performed in the initial encounter with global day period of 90 days and the patient returned after 3 weeks (within 90 days global period) of the surgery, it would be appropriate to code the service by appending modifier 78 which states "unplanned return to the operating/procedure room by the same physician following initial procedure for a related procedure during the post-operative period" with previously billed service for repair of blepharoptosis.

Thank you!
