My plastics surgeons do a lot of free flaps. They often have to go in and inject heparin into the anastomosed vein for venous congestion. Is there a code for this. I was thinking of using 35876-52, but not sure.

Please provide details of the procedure performed.

Patient had a DIEP flap with microvascular anastomsosis using superficial venous and greater saphenous vein graft. This anastomosis became thrombosed and heparin drip was given. Occasionally they also just go in and exam the anastomosis but most frequently they either us heparin or do a thrombectomy.

CPT code 35876 is not appropriate as this code should not be reported for the treatment of hemorrhage during the initial operative session nor treatment of postoperative hemorrhage not requiring return to the operating room. This code should generally be reported with modifier 78 indicating that the procedure represents a return to the operating room for a related procedure in the postoperative period.

Do you have any suggestions?????

Sorry I forgot to mention that I did not state a hemorrhage but a venous congestion with heparin injected to vein.

The appropriate code is 35875.

For heparin IV push CPT code 96374.

For IV infusion PT code 96365.
