Review and summarization old records

Bonnie Posted Mon 06th of June, 2016 12:18:44 PM
Hi- I have a physician who has documented in his progress note "CT head at hospital Xyz unremarkable. Repeat CT not fully visualized however no evidence of..." (CT head was done at a different facility prior to admission and Repeat CT was done at current facility on date of admission). Can we count the CT review done at hospital XYZ as review and summarization old records (2 points) and then the repeat CT as review in radiology section (1 point)? Thank you.
