Our client is pain management specialist. We are going to do institutional billing for the client. Which revenue code/s we need to add in UB04 before billing.

Revenue codes are four-digit numbers that are used on institutional bills to make the insurance company aware of either the type of services received by the patient, or the type of supplies and department in which services were rendered. Revenue code depends on the procedure code. Within most revenue code categories there are subcategories.

 

Please provide us the procedure code and place of service where service(s) were rendered. The provided information will help us determining the appropriate revenue code(s).

The above-mentioned procedure(s) are ASC approved procedures. For hospital-based ASCs, you are advised to report revenue code 0490 - Ambulatory surgical care. For every CPT® code(s) listed on the UB-04 form, the CPT® code for procedure(s) performed should be listed with a 0490-revenue code.
