The physician did a craniotomy for resection of skull base medial sphenoid wing meningioma. Would you code to site or type? '61519' or '61521'??

I would go for 61519 here.

For excision of meningiomas or tumors in other locations [other than the skull base], you would typically use either 61512, 61518, 61519 or 61520.)

Alternatively, while code 61521 (Craniectomy for excision of brain tumor, infratentorial or posterior fossa; midline tumor at base of skull) describes excision of a midline skull base tumor, this also reflects using an open surgical approach.
