As per the above presented documentation, the appropriate code for resection of osteoma of forehead will be CPT® code 61500- Craniectomy; with excision of tumor or other bone lesion of skull.

 

In this procedure, the provider removes a small portion of skull bone to remove a tumor or other abnormal growth of the skull.

CPT® code 21026 deals more with the facial bone anatomy. The facial bones include 14 bones, with six paired bones and two unpaired bones. The paired bones are the maxilla, palatine, zygomatic, nasal, lacrimal, and inferior nasal conchae bones. The unpaired bones are the vomer and mandible bones.

 

The forehead is a part of the cranial bone; i.e. the frontal bone; and, the frontal bone forms the forehead. Therefore, the most appropriate code for resection osteoma of forehead will be CPT® code 61500.
