Removal of shunt hardware and placement of an EVD

When coding a scenario where the externalized distal/value and the ventricular catheter was removed and replaced with an EVD that was inserted and brought out through a separate stab wound; would you code cpt codes 62256 and 61210-59 (new ventricular catheter exist through a separate stab wound)or would you code 62258; or just code 62256 and not code for the new ventricular catheter (EVD) placed? Please explain and provide references.

Well, I think coding CPT 62256 and CPT 62258 is not appropriate as there is no complete removal of CSF shunt system and CPT 61210 is for implantation of ventricular catheter. But in your case there is a replacement. So, CPT 62225 seems appropriate for the given scenario. Hope it Helps!

If the whole shunt was removed; why would you charge only cpt code 62225 which is just for the ventricular catheter replacement.

That’s correct, if we are removing the whole shunt and then replacing it with an EVD, we will bill with CPT 62258 only. Sorry for the confusion.
