I can only come across two codes i.e. 19110 and 19112. Where the CPT code 19112 can be coded only in case of lactiferous duct fistula, where else for the CPT code 19110 a suspicious/diseased lactiferous duct or papilloma lactiferous duct is excised. The dx supporting this can be either malignant/benign neoplasm, nipple discharge, or tumor. And if any of these CPT doesn't support then we can go for the unlisted 19499.
