Removal of a VP shunt placement of EVD wound debridement and revision

After a VP shunt is removed, an EVD placed,then a wound debridement and revision is done; wound flap is further debrided and sutured,would you code 62258 and what would you code for the wound debridement and revision? Also please list the diagnosis for left frontal wound dehiscence. Please list all codes for this scenario.

CPT 62258 should be used when complete removal of CSF shunt system with replacement by similar or other shunt at same operation. For wound, if the debridement is subcutaneous then as per the debridement depth/ area you can use code series 11042-11044 according to patient's record. Use ICD-9 998.31 or 998.32 as per procedure performed.
