Remicade infusion with outpatient H&P

A hospital here in our town is now requiring our GI physicians to perform a full H&P for all patients entering the hospital for outpatient Remicade infusions. Will this be a chargeable service for our physician or will it be considered global to the infusion service since the infusion is the service intended for the patient's outpatient hospital visit?

If this is only H/P , then it will be inclusive.

If your physician performed separately identifiable E/M services (have medical necessity) then it would be billed for physician.

This is also if our physician does not perform the injection, he is only ordering it. The injection is done by the outpatient facility at the hospital?
I assumed it would be, but wanted to be sure...
