rejected cpt due to J7308

Patient came in for CPT 99212 DX 702.0 and J7308 DX 702.0. Insurance only paid J7308. Rejected CPT 99212 "this service is not covered when performed on the same day as surgical procedure. We have always billed this way. Pt is seen by doctor and dr applies a product called Levulan. Insurance only paid for product and not the doctor's time. Is there another way to bill or did I do something wrong?

I prefer using Modifier 25 with 99212. Please refer this article.
