Reimbursement for amniotic membrane

The MD I work for saw a pt. with a corneal ulcer. He placed an amniotic membrane on the ocular surface for healing. Medicare reimbursed for procedure 65778 but not for the membrance itself which was billed with v2790. I have sent the actual purchase invoice w/patients name written on it to the payer. They deny saying info insufficient/incomplete. What am I missing? Thank you
