How would I code the following:
DIAGNOSIS:
1.Left recurrent cubital tunnel syndrome.
2.Subcu atrophy and skin hypo-depigmentation.
3.Medial epicondylitis, left.

PROCEDURE:
1.Excision, atrophic depigmented skin, which was 8 cm x 3 cm.
2.Submuscular transposition.
3.Medial epicondyle release with partial medial ostectomy, left.
4.Local transfer of subcutaneous fat over medial elbow, left.

For Medial epicondyle release with partial medial ostectomy code 24358, 11406 seems appropriate for depigmented skin excision and 20926 for transfer of subcutaneous fat.
Code ICD9 as- 726.31- Medial epicondylitis, 354.2 for cubital tunnel syndrome and 709.8 seems appropriate for hypo-depigmentation skin.
