rectosigmoid stricture with parastomal hernia

G Posted Mon 03rd of November, 2014 14:56:56 PM

OP report reads: Abdominal exploration, re-do low anterior resection (EEA stapled anastomosis), repair of right lower quadrant parastomal hernia. Billed as: 44145, 44346 - but per CCI edits not billable together. What should have been billed?

Thanks for your patience. In this scenario 44145; Colectomy, partial; with coloproctostomy (low pelvic anastomosis) is the only code that you should bill. 44346; Revision of colostomy; with repair of paracolostomy hernia (separate procedure) is not coded. Since by definition 44346 is a separate procedure, you are not allowed to report this code when the provider performs the service as an integral part of a larger procedure. This is why CCI prohibits billing both codes together under any circumstance. Hope this helps.
