Lately we have had a Medicare claim 93306 deny with a dx of 424.0. These cpt codes have paid in the past with the same dx code used before on other patients. The Medicare denial remark code states M25 N115. Could you provide us with any indication on why this claim is no paying.

The denial remark code M25 N115 indicate that the claim is getting denied due to coverage indications.

However, the coverage cannot be determined as we do not have complete information e.g. state of issuance.
