Reanastamosis of cranial nerve XI

Our surgeon did a reanastamosis of cranial nerve XI and states that he cannot find a code for this. Does a CPT or HCPCS code exist for this procedure?

Please look at the series 64866-64870.

I would prefer 64866 if your documentation matches.

Unfortunately, the surgeon tells me that none of that sereis of codes applies, because he did a REanastamosis of the nerve. I suggested billing 64866 with a modifier -22 and attaching the operative report, or do you suggest billing an unlisted code and sending the operative report. Either way, I am sure the surgeon won't be happy, but a code just does not exist for a reanastamosis of cranial nerve XI.

You are correct , there is no code to describe this procedure. I would prefer using an unlisted code here rather than the modifier 22. The OP should be well documentation of extra work done and payor may not get satisfied too with the reason. Use unlisted and compare it with 64866 rather.
