Ready to go for cataract surgery?

Pt was seen in Sept 2011 and cataracts were documented. Pt was not ready to have surgery. Pt called to schedule an appt in May 2012 as she states she is ready for surgery. Does Dr have to do another dialated exam if it was already noted in the chart in Sept that she had cataracts? Can I just do the H&P? Unable to find the coverage guidelines from Michigan Medicare.

The timing gap is near about eight months. Even though it was already noted in the chart in Sept, still doctor should do another dilated exam for further confirmation. I will search for coverage guidelines from Michigan Medicare and let you know if any findings I will be able to get.
