Re-do L3-L4 laminectomy for spondylosis

Need some guidance for coding a re-do laminectomy at L3-L4 where the original surgery was coded as 63047 and 63048. Would I use 63047 and 63048 again since it is a re-do for the same diagnosis for spondylosis or would I go with the re-do 63042. They are doing a decompression with partial facetectomy and foraminotomy at L3-L4? Thank you for your help.

CPT code 63042 is reportable for reexploration of laminectomy. If the initial procedure at L3-L4 was discectomy, the re-exploration would be reported with 63042 (Laminotomy [hemilaminectomy], with decompression of nerve root [s], including partial facetectomy, foraminotomy and/or excision of herniated intervertebral disc, reexploration, single interspace; lumbar) for (L3/4). However, if the original procedure was a decompression by laminectomy and facetectomy, the re-exploration would be reported with the same 63047 and 63048, perhaps with the -22 modifier to reflect any significant additional work if any documented.
