Hi,

I would suggest you use CPT code 64635 for radiofrequency Neurotomy performed at L5 and CPT code 64636 x 3 units for Neurotomy performed at S1, S2 and S3 levels.

CPT code 64635 suggests destruction performed at lumbar or sacral level. Hence we can code 64635 and 64636.

There is a change to the answer provided to you. You should bill 64640 four times for the four radiofrequency ablations performed at dorsal rami of L5, S1, S2 and S3
