Patient is being seen in the radiation oncology clinic for adjuvant therapy after having mastectomy of the right breast with no evidence of disease. What is/are the appropriate diagnosis code/s?

For encounter of radiation therapy, we must bill V58.0 as a primary code. And as mentioned in query that there is no evidence of disease, I would suggest to bill V10.3 as secondary to show personal history of neoplasm malignant of breast.

Yes sure, you can bill Z51.0 instead of V58.0 as primary and Z85.3 as secondary instead of V10.3
