Radial and Median and Ulnar nerve blocks

Physicial performed a radial, median and ulnar never blocks on the right hand with Ultrasound guidence.

Would I code 64450 Radial, and 64450 Medial and ulnar block. (The doctor reposition the needle after the Ulnar nerve injection to inject the Median nerve.)

Diagnosis used was neuritis of the radial ,median and ulnar nerves. would code G56.31, G56.11 and G56.21

As the physician performed on three nerves cpt 64450-RT is correct for each nerve procedure. Diagnosis G56.31, G56.11 and G56.21 are correct as per the findings.
