question related to return visit after a biopsy

If we have the following scenario, how can we bill for the return visit:

new pt comes in sees our ENT and a biopsy is done that carries a 10 day global, the pt is scheduled for a return visit with our head and neck ENT surgeon. THe biopsy is malignant, the return visit is very lengthy discussing options, alternatives, etc..........can this second visit be billed if it occurs within the global period? Can a 24 modifier be used? I am thinking not because it is related but I am not sure.

You cannot bill an E/M for anything related to that procedure within that Global post op time frame. Regarding modifier 24, I agree with you that it cannot be appended in a related visit. Modifier 24 can be appended only in a global period for a visit that is separate and distinct from the expected postprocedural follow-up.
