Could a hospital in network billing contract differentiate any of the above questions? Sometime the hospitals have direct contracts with carriers/PBM's.

Billing of Lidocaine Hydrochloride or Zingo depends upon payers/carriers. Some carriers/payers pay for it. Most of the carriers/payers do not pay for zingo. These codes are for items and/or services that CMS chose to exclude from the fee schedule payment by regulation. Also you need to check with your payer if they allow billing zingo.
