Q0138

Loni Posted Wed 13th of August, 2014 14:58:13 PM

When billing for Feraheme (Q0138) in patients with Iron-Deficiency Anemia, MUST a diagnosis of CKD (585.1-585.5) be documented, or will the Anemia alone suffice? I cannot find documentation of this requirement as a resource.

To bill Q0138 two diagnosis codes are required. For information regarding approved diagnosis codes see http://apps.ngsmedicare.com/SIA/ARTICLE_A48420.htm

Hope this helps.
