PTA w cath exchange

Discussion:
The site was prepped and draped in the usual sterile fashion. Local anesthetic was administered around the catheter insertion site. The subcutaneous cuff was released from surrounding soft tissues using blunt dissection. Two glide wires were advanced through both the ports of the catheter into the inferior vena cava. The existing catheter was then removed and a new long term catheter was placed through the tunnel over the wires under fluoroscopy. The catheter was secured in place using a non-absorbable skin suture. Both ports were flushed with saline followed by heparin lock. Patient tolerated the procedure well. No immediate complications were encountered.

New catheter placed: Prior to removing the catheter a superior venocavagram was performed through catheter and this identified a high grade lesion and fibrin stheath present in SVC. Next a 10X4 balloon was taken to area and PTA carried out with less than 30% remaining stenosis Next new catheter was exchanged and placed a 23cm via fluoroscopic guidance.

I understand but I am just wanted to make sure that the codes we billed noted above are correct with what was noted not for you to code it I did provide the codes.

To verify the codes billed are correct, requires complete review of the operative report which qualifies it for SOD service. Kindly get in touch with SOD.

oh ok you have help before when I gave the codes and op note to determine if it was correct so did not know it was not allowed.
