what is a proposed LCD & How many days does it take an proposed LCD to become active coverage policy. Which part of the year does LCD change happens ?

Proposed LCD - Proposed LCD is a draft LCD policy which has been created but not yet activated, but will be activated soon when the complete data will be available.

Time (Proposed to active policy) - There is no specific period of time mentioned by CMS. Sometimes it takes a week or month or sometimes those policies get retired even before activated.

Update frequency - CMS updates LCD every week.
