Hi is it appropriate to bill add on code 44139 mobilization of splenic flexure when there is extensive malignant disease and there is debulking involved or in what manner would it be billable thanks garmaba

Debulking involves removal of multiple surrounding tissues, but as the CPT code 44139 is an Add-On code for mobilization of splenic flexure, then it would be billed separately along with the primary code(s) of colectomy.

yes what if there was no colectomy performed only debulking and the provider needed to do extensive mobilization of the splenic flexure. thanks-

CPT code 44139 is an add-on code, which can only be billed in conjunction with 44140-44147. If mobilization of splenic flexure was done during debulking and without colectomy, then we left with the unlisted CPT code only. While reporting the services with Unlisted CPT code, it is highly recommended to provide the appropriate documentation which state that there is no specific CPT code available for performed procedure/service(s).
