I would like to know the appropriate way to bill;
65750, 67010, 66986

According to CCI edits, You can bill CPT code 65750 and 66986, but CPT code 67010 showing bundling in CPT 65750. If vitrectomy is not incidental and your documentation supports the procedure, then you can bill it with 59 modifier.
