Is this correct ICD coding:

ASSESSMENT:
Early intrauterine pregnancy at 11 weeks, 36 yo primigravaida.
Chronic medical problems with hypertension.
Diabetes.
Chronic renal disease.
Hypothyroidism.
On dialysis.

I come up with the following codes:
648.03
250.40
403.91
5856
V45.11
V58.67

Is this correct?

Please help.

648.03,642.93,646.23 are the first three codes to be used.
