pre surgical screening

when a pt. came in for surgical screening and chronic disease was also attended to. how can i code that visit? can i put the v72.84 as the primary DX code and the chronic DX. code. please help to clarified this gray area.

As per ICD-9 guidelines we should code reason for visit as a primary code. So in this case V72.84 will be the primary dx followed by the chronic condition and reason for surgery.

What about if the reason for surgery was not documented?
