What code is appropriate for a pre-op visit for a patient new to a pratice? Would "99243-99245" be appropriate for non Medicare patients? Would "99203-99205" be appropriate for Medicare patients? Would "99213-99215" be appropriate for established patients requiring pre-op clearance? Is the Dx code V72.83 appropriate? Thank

I agree with you, the answers are there in your questions. The ICD code range should be V72.81-V72.84 as applicable.
