If active warming and/or at least one body temp equal to or greater than 96.8 degrees Fahrenheit was taken within the designated timeframe for a general or neuraxial anesthesia case that is less than 60 minutes do I report G9363 only?

Yes, you can use Denominator code as G9363. But you cannot use any Numerator code or combo from Measure#193 in this case.

What code would be used if the time is under 60 minutes but active warming and or the pt's temp wasn't taken so the measure numerator wasn't met?

Use CPT code 4250F with 1P modifier - Intentional hypothermia OR active warming not indicated due to anesthetic technique: peripheral nerve block without general anesthesia, OR monitored anesthesia care AND HCPCS code G9363 - Duration of monitored anesthesia care (MAC) or peripheral nerve block (PNB) without the use of general anesthesia during an applicable procedure or general or neuraxial anesthesia less than 60 minutes, as documented in the anesthesia record.
