Posttraumatic Wrist Arthrosis

How would I code the following:
DIAGNOSIS: Right wrist posttraumatic arthrosis with failed scapholunate repair.

PROCEDURE:
1.Posterior interosseous nerve neurectomy, right.
2.Removal of retained compression screw.
3.Right scaphoid excision and four corner fusion with local bone grafting from removed scaphoid and from the distal radius.

I know 64772 for the PIN (procedure 1), but am unsure of the rest.

I would suggest appropriate cpt 20680 for Removal of retained compression screw and 25810 for Cpt scaphoid excision and four corner fusion with local bone grafting from removed scaphoid and from the distal radius.
