CCI edits shows bundling of CPT 64415 with 36821. So, it cannot be billed together.

Our anesthesiologist also perform post-op interscalene nerve blocks for pain relief on patients for rotator cuff procedures. We were told we could also bill for the blocks as long as the blocks were requested by the surgeon for post-op pain relief and as long as we put a 59 modifier with the block. Does this not apply to the fistula?

Yes, You are right, You can bill CPT 64415 for interscalene block in this case. But CCI edits shows bundling of CPT 64415 into 36821. you can give it a try by bill CPT 64415 with 59 modifier.
