pleural catheter exchange with sinogram

Diana Posted Tue 30th of July, 2013 22:00:03 PM

Since there is not code for exchange of this catheter, I want to charge 32556. Can I also charge for the sinogram? or would that be inlcuded in 32556.

REMOVAL OF PREVIOUSLY-PLACED 12-FRENCH PLEURAL DRAIN 2.
PLACEMENT OF NEW 12-FRENCH PLEURAL DRAIN WITH SINOGRAM

CLINICAL INDICATION:
The patient has empyema with chronic pleural fluid collection. The patient has a sinus tract extending from the skin to the pleural collection and has had a drainage catheter within this.

PROCEDURE:
With the patient in the prone position, the skin over the left posterolateral chest wall and previously-placed catheter are prepped and draped in the usual sterile manner. 1% lidocaine is used for anesthesia. A 0.035-inch wire was advanced through the previously-placed pigtail catheter and the catheter is exchanged, leaving the wire in the pleural space. A new 12-French pigtail catheter was advanced over the wire. The pigtail loop was formed and locked within the pleural space. Contrast injection with 10 mL of Visipaque shows contrast within the pleural loculated collection. The catheter was sutured to the skin and a sterile dressing placed over it.

IMPRESSION:
Exchange of pigtail pleural catheter.
