Please help me with the coding

Our doctors performed a diagnostic cerebral angiogram. They injected the left common carotid, right common carotid, right external carotid and the right vertebral artery. They then injected the right transverse artery, the sigmoid junction, the left transverse and the left internal jugular vein. An angioplasty was performed on the left straight sinus. Then an angiogram was performed to see if the procedure helped and it did. Thank you

For diagnostic cerebral angiogram, select code from 36221-36228 series depending upon your documentation. And for angiogram, select code from the following: 36902, 36905, 36907, 37246, 37247, 37248, 37249 based on your documentation.

Here are the codes that I submitted. 36226 RT, 36223 (50), 36227 RT, 37248 for the angioplasty, 36012 injection of the internal jugular vein, and the 75870. Please tell me if these codes seem correct.

Though your coding seems correct depending upon this limited documentation, but please note that code 75870 has CCI edit with codes 36226, and 37248. Code 75870 is a column 2 code for these codes, this may be reported only under appropriate circumstances with appropriate modifier. Please check detailed medical documentation for final reporting.

Please specify in which circumstances I can code the 75870 code and which modifier needs to be used. I coded the 26 and 59 modifier and the code was denied by the insurance company.

This can be reported where modifier 59 is applicable, e.g. separate injury, separate surgical operative session on the same date of service, etc.
