As per the above provided documentation, instead of reporting an Unlisted code; there is a more appropriate code for Z-lengthening of quadriceps tendon; i.e. CPT® code 27685 (Lengthening or shortening of tendon, leg or ankle; single tendon).
In this procedure, the provider makes an incision down through subcutaneous tissue to the tendon sheath of the affected tendon. If the tendon needs to be lengthened, he makes a Z shaped incision to lengthen the tendon and asks the patient to move the foot to check adequate length. To shorten the tendon, the provider cuts away the extra length of the tendon. In both cases, the provider reapproximates the cut ends with sutures in an end to end repair. He repairs the tendon sheath with sutures. He applies a short leg walking cast.
Note: Arthrotomy, left knee, with lysis of adhesion will not be reported separately, as it will be considered integral to the Z-lengthening of quadriceps tendon.
