37195 is a PC/TC Indicator 5 Code.
Now, we need to understand the concept of PC/TC Indicator 5 Codes:-

Effective for claims with dates of service February 01, 2011 and after, many Insurance companies deny
“Incident To” codes identified with a CMS PC/TC indicator 5: http://www4a.cms.gov/transmittals/downloads/R2119CP.pdf
CMS PC/TC indicator 5 = Incident to codes: This indicator identifies codes that describe services
covered incident to a physicians service when they are provided by auxiliary personnel employed by the
physician and working under his or her direct supervision.
Payment may not be made by carriers for these services when they are provided to hospital inpatients or patients in a hospital outpatient department. Modifiers 26 and TC cannot be used with these codes.
