Please let me know what you would code for a halo pin debridement, removal of a pin and replacement of another pin. I thinking 20693 for the adjustment; however, need a debridement code. If the pin was cleaned and debrided using cotton tip with Hibiclens, what would you code for the debridement. Also, would you code 839.01 for atlantoaxial rotatory subluxation if this was a return visit? Please list the cpt codes and diagnosis associated with this scenario.

Well, I don't think that we need to code the service that you are describing, it is a part of the adjustment code that you are planning to bill (CPT 20693). And ICD is also correct for atlantoaxial subluxation i.e. 839.01. Hope it helps!

I thought you could charge separately for the debridement. Is there a reason why you can't charge? If so, please indicate why. Please list resources.

Well, clinically for fixation of external device it is mostly required to debride the surrounding structures to make out the pathway for core location and to clean the area and to remove the debris. So, debridement will not be coded separately in this scenario. Hope it helps!

Please reference your reader question: Verify 20693 OR 20999 for halo vest adjustment in a Super Coder Orthopedic coding alert. There it says that you can code the debridement separately with 11040; however, cpt code 11040 has been deleted and I need the another code that would best describe debridement; if the pin was cleaned and debrided using cotton tip with Hibiclens, what would you code for the debridement?

Well, according to your base query "pin was cleaned and debrided using cotton tip with hibiclens, which is not to be coded. But, after your last post it seems that the debridement is subcutaneous. So, as per the debridement depth/ area you can use code series 11042 to 11044 according to patient's record. Hope it helps!
