"If a Medicare patient is seen by our physician for an annual wellness visit or a subsequent annual wellness visit and answers YES to Depression Screening questions or Functional Ability/Safety Screening for Home Safety and then we ask them on the same date of service to complete the Home Safety Questionnaire and/or the Patient Health Questionnaire-PHQ-9 and our physician reviews and discusses the patient's answers with them, are we able to bill a separate code for the time spent giving and reviewing the questionnaire(s)in addition to the AWV codes? Thank you.

No. This would be part of the Review of Systems [R.O.S.] and part of the E/M visit.
