The correct diagnosis code for initial encounter of condition (Subluxating/snapping right peroneal tendons) is S86.391A. The procedure CPT will be 27675 if surgeon has performed peroneal tendon repair without fibular osteotomy, if surgeon has performed peroneal tendon repair with fibular osteotomy then use CPT 27676. For correct CPT code check documentation of operative report.
