I have a diagnosis coding question about "recurrent high grade mammary carcinoma infiltrating through the muscle and fascial connective tissue". Should this be a breast cancer diagnosis or a metastatic to the muscle diagnosis.

The pathology does not say metastatic it says the phrase above. Thanks for your help.

This would be a "primary malig. neoplasm of breast" Dx. The code should be 174.9, as I don't see any specific part of mammary gland mentioned in the query. The usage of word "recurrent" does not infer any Secondary malignancy and metastasis is clearly not mentioned.

Thanks so much that is what I was thinking but I wasn't sure.
