Path report reads Bronchial and perialveolar parenchyma with focal intra-alveolar pulmonary pigment-laden macrophages. (Right upper lobe biopsy) Provider did a Diagnostic bronchoscopy.

How do you code this are can it be coded?

As focal intra-alveolar pulmonary pigment-laden macrophages is a histological findings and normally found in bronchiolitis; but we can't assume that. As this finding is from a path report we can not correlate and assume any diagnosis out if. The provider is the best person to establish diagnosis out of it. Request you to please review the documentation thoroughly and find out if there something mentioned regarding bronchiolitis, if not, try using other specified code. But in such situation coding sign and symptoms is most recommended as it is a diagnostic procedure and should support well for payment.
