Patching Therapy

I have a patient who the OD is initiating patching therapy (to correct for a lazy eye, or amblyopia). I don't think medical or vision insurance covers patching or vision therapy, does it? And if not, why is it acceptable to bill a 99 E/M code with amblyopia as the diagnosis? What's the distinction being made?
