Pars plana vitrectomy w/removal of silicone oil

What CPT and DX code would I use for a PPV w/removal of silicone oil possible replacement of silicone oil? I was thinking 67036 w/67121. Also, the patient is having endo ciliary body destruction would I use 66711 for that CPT?

You would use the following codes.

67036-59 PPV
67121 Implanted material removal
66711 Ciliary body destruction
