Is there a code for a pap smear done by a provider. Only the pap was done and the patient does not have Medicare it is a private insurance? , HOW DO I BILL THIS? (no preventive, because the internist already used a preventive code)

If the service is a screening service, that is, part of an annual exam or preventive medicine service, then there is no additional code to bill . The preventive medicine codes include an age/gender appropriate history and exam, along with anticipatory guidance, risk factor reduction and referral for or provision of immunization and other screening diagnostic tests appropriate for that patient's age, gender and medical history. That is, if the woman's age requires a pelvic and pap smear, those services are part of the payment for the preventive medicine service. There is no additional payment. And, you are not paid less for not doing it, if it is not required."

As per CPT guidelines, there is no separate CPT code for Pap smear. You will not get any pay for this service separately. In this scenario, internist already billed for preventive service, its means internist already billed this service.
This is inclusive service with preventive service whether it is done or not. Physician or internist are not paid less for not doing this.
