The Dr has written " Therapeutic Greater Occipital Nerve Rhizotomy on the Left " (same on the right)He used radiofrequency and a "lesion was carried out...".
I need help with the CPT code for this. 64415 or 64999?

You should report 64640, Destruction by neurolytic agent; other peripheral nerve or branch, for Greater occipital nerve rhizotomy.

You may bill two units of this code for procedure performed on right and left side.
