Pacemaker to ICD upgrade with DFT

Dx: I49.5
Patient presents for replacement and upgrade of his single chamber VVI pacemaker pulse generator to a VVI-ICD.
He has CHF with LVEF <35% but is not pacemaker dependent and QRSD <150 ms.
His device is upgraded to a single chamber cardiodefibrillator VVI-ICD.
The existing RV pacemaker lead is removed by transvenous extraction with a mechanical Spectranetic sheath in the Hybrid room and a new RV defibrillator lead is inserted.
The old pacemaker pulse generator is removed and a new single chamber ICD pulse generator is inserted, and connected it to the new RV defibrillation lead.
Defibrillation threshold is done using the new RV defib lead connected to the new ICD device.

CPT: 33249, 33241, 33243, 93641

Are the codes correct?
Is the code order adequate?
Any modifiers to be included?

I think the codes will be 33249, 33233, 93641-26.

I could not see thoracotomy in documentation mentioned by you.
